Service Request

Details for the service form are:

Priority: URGENT. Lexmark Optra E312 Laser Printer, Serial No 486322180.

The printer will turn on, however nothing will print, including a test page.

I have checked the printer cables – they are connected correctly. The cartridge has also been inserted correctly.

Save the form into the HR\Service Requests folder with the filename SR10 and print a copy.

As Lyn will be away, sign the form on her behalf and personally give it to the Administration Manager ASAP.
